I authorize Wreck-A-Mended Auto & Body, hereafter called WAB, to perform all necessary repairs to my vehicle. Repairs are done per WAB estimates.
Repairs to my vehicle will include parts, materials, tear down, diagnostic, and labor. If upon further inspection, additional parts and/or repairs are needed. WAB will notify the insurance company (if any), and the customer for authorization. We are not responsible for any damages from fi re, theft, or any other causes beyond our control of the vehicle or any articles left in the vehicle. Any tests made by WAB employees are done at your own risk. If the vehicle is returned to the customer without any repairs being done, there will be a diagnostic charge, and other charges may apply. An Express Mechanic’s Lien is acknowledged on the vehicle listed above to secure payment on repairs and fees.

Attention: All Texas residents and/or visitors, upon completion of repairs of this stated above vehicle may be subject to repossession under the Texas Property Code 70.0001 (B) for non-payment of any kind.

Express Power of Attorney
I authorize WAB the facility where my vehicle is being repaired, to endorse any insurance check which pertains to my vehicle listed above. The purpose of this
is to avoid any delays on purchasing parts and to continue repairing my vehicle

non-stop until completion.
